Our office hours are Monday –Thursday from 8am to 5pm and Friday from
8am-3pm. If you have an emergency after hours or on weekends, please call to be forwarded directly to Dr. Lener, Dr. Litchfield, or Dr. Khoury.

If you have any questions when filling out the form, please do the best you can, our staff will assist you with your questions on the day of your visit. Please fax your form back to us at 949.364.8511 or arrive 10 minutes early with your completed form.

New Patient Registration Packet

Insurance Policies

Physicians at The Dermatology Center at Ladera accept appointments for patients regardless of their insurance status.  We participate in most PPO plans, Medicare, and Tricare.  Patients without insurance will be asked to pay our office directly for their care on the date of service.  We have set our fees fairly so that patients who value our care can be treated at reasonable rates.

In-Network Provider/Insurance Accepted:

• Medicare
• Most PPO plans - see exceptions below

For patients who are covered under of one of the plans in which we participate, we will ask for any co-pay in advance.  In some exceptional cases, we will also ask for coinsurance or deductible prior to treatment.

Out of Network Provider/Insurance Not Accepted:

• Medi-Cal / CalOptima
• HMO insurance
• United Healthcare/Pacificare
• HealthNet
• Great-West

Out of Network patients will be asked to pay our office directly for their care on the date of service, then get reimbursement from their insurance.  It is our recommendation that patients check with their insurance company about benefits for out of network providers, so that there are no financial surprises.  Sometimes there is a separate deductible or other payment differences.  We have many patients that are cared for on an out of network basis and are pleased with how it is handled at our practice.
